Health Equity Toolkit

Building a Culture of Health & Promoting Health Equity

This unique set of tools, designed for nurses and various professionals within the healthcare cohort, were manufactured to create, improve and maintain a culture of health.

Defined by The Robert Wood Johnson Foundation, a culture of health puts priority of well-being in multiple facets of everyday life. This toolkit is based on the process of nursing: assessment, diagnosis, planning, implementation, and evaluation. By using the tools and resources provided by the Future of Nursing: Campaign for Action, inter-professional partners can access action based strategies and concrete steps for individuals, communities, and health care facilities to achieve a standard of effectiveness in health and well-being. The ADPIE framework assists health care professionals of all education and experience level in evaluating the willingness, enthusiasm, and preparedness to enhance health equity and diminish social determinants of health in the communities they serve.
